In order to register online, send Nancy Elling, the coordinator, an e-mail with a message stating your intent to participate in the next clinic. Don't forget to include the date.
The planned clinic dates are:
April 26,1997
July 26, 1997
October 25,1997
January 24,1998
Clinics are on the fourth Saturday in January, April, July, October.
On the day of the clinic, please bring a printed copy of the release (available on this site), the questionnaire that Nancy will send you as soon as you register, and any copies of test results and films such as CT scans, MRI, EEG and ultrasound examinations. If you have problems downloading the release form from this site, NTSA or Nancy Elling will be able to send a copy by mail along with the questionnaire. It is very important that Dr. McClintock and the other physicians get an accurate picture of your or your child's condition. We will return all tests.
